What we do

YOU has been providing services for people with disabilities for 30 years and has a strong track record of helping people achieve their goals. We aim to meet people’s care or support requirements by providing a range of services tailored to each person’s needs and aspirations, whether in one’s own home, in the community, by way of advice and information or in shared supported living or residential care.

We do this by putting the person at the centre of all we do, being flexible and adaptable in our support and ensuring that we work alongside the person, their families and carers to promote the best outcomes.

All our care services are CQC registered and our guiding principles include listening, engaging, reflecting and involving people to walk alongside them empowering individuals to maximise choice and control over their own lives and release their full potential to live the life they want.

Our work can only be successful when we create open and honest communication and truly show respect and dignity with everyone we meet.

Community Support aims to provide people with learning disabilities with information and advice to lead independent lives through empowering people to make their own choices. Staff assist people to access and develop natural social networks within their community. Through co-produced actions plans, our aim is for individuals to access their community with minimal or no support from staff by increasing an individual’s confidence and independence at a pace suited to them. Some of the areas we can support people with are:

  • Having a say in their community
  • Developing peoples daily living skills such as good neighbour skills or talking through any problems a person may have.
  • Claiming welfare benefits.
  • Financial budgeting.
  • Attending appointments.
  • Accessing education, training or work.
  • Joining community groups.

Into Work Services – Portsmouth

The Into Work service aims to help people with a learning disability to build their confidence and independence to overcome any hurdles people face in getting a job. The service can assist people in applying to jobs, voluntary and paid placements, in achieving your own work goals. We also aim to work closely with all employers who employ or are thinking about employing people with a learning disability.

Into Work services explore with individuals their work aspirations to determine work options and with providers to increase the number of options for people. To increase achievements and outcomes we’ll work alongside people, shadowing where appropriate and give on-going encouragement and feedback, until a persons confidence and strengths increase.

Dorset

The Housing and Supported Assessment Service works together with those using the service and their support networks where appropriate. The service assists in guiding people in developing the skills they need to live on their won, such as cooking, budgeting and shopping. People’s wishes are paramount and are placed at the centre of all support. Our aim is to assist people in moving on to new accommodation by supporting them to access the housing register, bid for properties and help with moving arrangements. The service undertakes assessments and can advise people accordingly of the affordability of rent (including service charges) charged to people. Service staff work alongside people in exploring options about different funding streams, including direct payments, personal budgets and individuals service funds, and ensure people are aware of the impacts of options.

YOU is committed to a person-centred, co-produced service, working together with people to make sure people are receiving the service they want. People are included in every aspect of living at one of our supported living services, for example, recruiting new staff, how people receive support, to making decisions bout the building such as decorating, throwing parties and activities. YOU want people to have control of their lives and YOU are here to help people achieve that. Supported Living Services can offer:

YOU offer person-centred, safe and friendly Registered Care Homes for people with learning disabilities in Hampshire. We place the individual at the centre of their support and they are supported by a team of highly trained specialists, motivated on delivering a quality service. Support is co-produced between staff, people and their support networks (where appropriate). We are funded by Hampshire County Council and our Registered Care Homes complies with CQC guidelines an regulations. Registered Care Homes offer:

Basingstoke Launchpad

The Launchpad service offers community based leisure, occupational and vocational activities across Basingstoke. This service is intended on providing day and leisure activities to those who had been living in house day services previously. Small groups are supported to participate in an activity of their choosing, with people s confidence increasing as our aim.
