- Health Coaches
They are employed on behalf of some innovative GP Practices. These are based inn Portsmouth & South East Hampshire. However, other GP practices employ their own Social Prescribers directly. If you are a patient and would like to speak to a Social Prescriber, then please ask the receptionist at your GP surgery to make an appointment.
We have a number of teams working with GP Surgeries specialising in Social Prescribing, Health Coaching, and Care Coordination. We work with people to find out what is important to them. So we can link them with all the great things they want to do that will have a positive impact on their physical and mental health.
Social Prescribing Partners
Our Social Prescribing Partners focus on what matters to someone. Like building trusting relationships with people to find the root cause of issues. These could be welfare, housing or social Isolation. We connect them to community groups, VCSE organisations, and services that can help.
Social Prescribers also work with the community to develop new social opportunities and groups.
Open water swimming is a great way for people to come together share in a new experience.
Health and Wellbeing Coaches
Our Health and Wellbeing Coaches deliver one-to-one coaching sessions, creating personalised action plans centred on a person’s own goals and targets. They also deliver tailored ‘Lifestyle as medicine’ peer groups to support people in making positive changes to their physical health and mental wellbeing through exercise, increased activity, and mindfulness.
Young Persons Social Prescribing – Hayling & Emsworth
Young Persons Social Prescribing aims to support individuals between the ages of 11 and 25. We help with a wide range of social, emotional or practical needs whilst improving mental health and physical wellbeing.
Our aim is to connect young people to community groups and statutory services to meet their emotional and social goals. This includes taking the time to find out what’s important to them right now. That might mean addressing social isolation. This is done by building connections, introducing and accompanying them to community activities and groups. Or connecting to lower-level mental health support.
Our team understands that whatever the specific nature of their circumstances, individuals deserve the respect with their own unique needs and interests.
Care Coordinators
Care Coordinators ensure that people receive the coordinated care they depend on, which is responsive to their needs and co-produced with them.
Care Coordinators act as the go-to person if someone’s needs change or if there are questions relating to their care. They are the key point of contact for professionals working to support someone.
