Essential Requirements
The appointed Hospital Discharge & Community Support Worker will:
- Have relevant experience in healthcare or community support roles;
- A passion for improving patient outcomes and community well-being;
- Willingness to undergo training and continuous professional development
- Be caring, consistent, flexible and creative, work well under pressure, know how to connect with people at all levels
- Have a broad skills and knowledge base including knowledge of housing related support; housing law; debt; welfare benefits; welfare reform and universal credit
- Have experience of working with clients to produce strength based support plans and positive risk management
- Have experience of working with a diverse range of client groups
- Have knowledge of safeguarding for adults, children and young people
- Be able to prioritize own time / workload
- Be able to keep up to date and accurate case notes
Due to the nature of the role the Hospital Discharge & Community Support Worker you will need to hold a full driving license with access to a vehicle

The Role
As a Hospital Discharge & Community Support Worker you will be responsible for:
- Managing your own caseload, providing one to one support out in the community and in a hospital setting.
- Meet 1:1 with clients, in hospital, their accommodation or another venue of their choosing, in order to provide tailored person centred support, promote their independent living skills, health and wellbeing, and develop greater resilience
- Providing practical support for example navigating an individual through a housing or benefits application
- Encouraging individuals to gain access to resources that they may not have been aware of that support positive health and wellbeing for example to addiction services or connecting to a social group or activity
- Supporting people to find long term solutions that will promote independence
- Supporting clients to access suitable long-term accommodation and any support with resettlement from hospital.
- Partnership working with a range agencies and organisations, often advocating on a client’s behalf

Be a crucial part of the healthcare journey, helping patients transition smoothly from hospital to the community when they are medically fit for discharge and need extra support to return home safely, both the emergency department and from the wards.
- Have the ability to connect with individuals during vulnerable times and provide genuine support
- Able to thrive in a fast-paced environment with the flexibility to handle diverse situations
- Someone with strong verbal and written communication to liaise with various stakeholders effectively.
- Be able to work flexibly in the East Dorset area and Purbeck be able to travel with in the county depending on the needs of patients.
This role does not include the delivery of personal care.
