The appointed Hospital Referral & Discharge Coordinator will:
- Have an awareness of the social factors that can delay a persons discharge from a hospital setting when they are medically fit to return home.
- Have a broad skills and knowledge base including some knowledge of housing related support; housing law; debt; welfare benefits; welfare reform and universal credit
- Have experience of working with a diverse range of client groups
Due to the nature of the role the referral and discharge coordinator will need to hold a full driving license with access to a vehicle

The Role
If you enjoy working in a hospital setting alongside our community partners then this is the role for you.
The aim of the service is to support a safe and timely return home for people who no longer meet the clinical criteria to reside in hospital or do not need to be admitted for clinical reasons but there are environmental or social concerns about them returning home alone.
As a Referral and Discharge Coordinator, you will be based at Dorset County Hospital Dorchester, you will be responsible for:
- Accepting referrals from all Dorset Hospitals, Emergency Departments (ED) GPs and Urgent Community Response Scheme
- Assessment and allocation of a First Point Discharge and Community support worker to complex cases
- Supporting clients at the first point of contact to resolve barriers to discharge e.g Social isolation, property condition or environmental issues within the home
- Working collaboratively with all Dorset Hospitals (including community and acute provision) working closely with hospital and social work teams including but not limited to ward staff, discharge teams, ED teams, cluster teams, patient key workers and hospital volunteers, and social workers.
- Providing practical support for example navigating an individual through a housing or benefits application,
- Encouraging individuals to gain access to resources that they may not have been aware of that support positive health and wellbeing for example to addiction services, peer support or a social group
- Partnership working with a range agencies and organisations, often advocating on a client’s behalf
