Social Prescribing Partner & Partner for Children & Young People
The Social Prescribing Partner empowers people to take control of their health and wellbeing by giving time to focus on ‘what matters to me’.
You will have experience of working in health, social care, voluntary sector or information and advice, in direct contact with clients, enabling people to achieve positive outcomes.
You will have the ability to organise service administration to comply with GDPR
You will have knowledge of the range of voluntary, statutory, charitable and private sector organisations in Portsmouth.
Able to use initiative and think creatively to achieve client goals, including the use of volunteers
Good communication skills: verbal, non verbal, and written.
Be able to travel around the local area with flexibility to be able to work very occasional evenings, weekends, bank holidays.
Confident user of the Microsoft office package

We are recruiting for two new Social Prescribing Partners, and one of these roles will focus on Children & Young People. Their role is to support Children & Young People & their families as well as people leaving the care system and Young Carer. Connecting them to support and strengthening communities.
The Social Prescribing Partner empowers people to take control of their health and wellbeing by giving time to focus on ‘what matters to me’. You will build trusting relationships with people, create a shared personalised care and support plan and connect them to community groups, VCSE organisations and services. You will also work with partners to provide support to community groups and VCSE organisations involved in social prescribing.
This approach particularly helps people with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social issues which affect their wellbeing.
You will also link people to our free to access projects funded by the National Academy of Social Prescribing for this PCN – Cycling Without Age, Arts Projects, Cooking Classes, Photo walks & Sea Swimming for Mental Health courses.
Work within the primary care network multi-disciplinary healthcare team, providing 1:1 personalised support to people who are referred to them by team members and local agencies.
Help people to work on their wider health and wellbeing, specifically wider determinants of their health, such as debt, poor housing and physical inactivity, as well as other lifestyle issues and low level mental health concerns by increasing people’s active involvement with their local communities.
While acknowledging face to face support and real world connection is ideal, you will work with people creatively to overcome digital barriers, supporting them to connect to their family, friends & community, digital social opportunities and health & wellbeing services now available online.
You will contact referred patients on the phone/ MS Teams/ meet them in the GP practice/ in their home or other community settings. You will listen, empower and motivate, understanding the root cause of problems.
You will liaise closely with GPs, Health Professionals & practice staff across the PCN acting as part of the PCN/ General Practice team and a wider multi-agency integrated team. You will also explore and build on knowledge of the health and social care and voluntary organisations/services available in the area, identifying and contributing to the development of community sources of support.
You will plan and manage your own diary and caseload on a day to day basis, with time spent between individual PCN GP Practices, community settings, home visits and casework admin.
You must be a good listener, have time for people and be committed to supporting local communities to care for each other.
You should have experience of working positively with people facing complex social and emotional challenges.
You must be able to build and maintain positive working relationships with internal and external contacts

Brunel Healthcare PCN is a large partnership of GP Practices in Portsmouth with a very diverse population working together to improve the care for their population. Brunel Healthcare PCN have chosen to partner with the YOU Trust, who deliver an area-wide Social Prescribing Service, Care Coordination, Health Coaching and the team are experienced in these newly emerging roles. This role is based in the Community Services Team in Portsmouth. As a YOU Trust PCN Social Prescriber you will join this team to have access to the latest training and support available for an exciting and valuable role with opportunities for progression.
